What is IHS?
At Covenant Human Services, our Individualized Home Supports (IHS) program is a Minnesota DHS 245D-licensed service designed to provide flexible, person-centered support to individuals living in their own home or with family.
We focus on meeting each person where they are—building services around their goals, preferences, and daily life, not the other way around. Whether someone needs occasional assistance or ongoing support, our approach is always individualized, practical, and empowering.
Who Can Benefit from IHS?
IHS is ideal for individuals who want support with:

Building daily living skills

Receiving evening, overnight, or weekend support
Who Qualifies?
An individual may be eligible for IHS if they:

Live in their own home or a family home
-
Receive services through a qualifying Minnesota disability waiver:

Brain Injury (BI)
-
Community Access for Disability Inclusion (CADI)

Developmental Disabilities (DD)

Have a case manager and an approved support plan that includes IHS
Case managers coordinate service authorization and planning.
Our IHS Service Options
We offer three types of Individualized Home Supports:
IHS Without Training
Support for individuals who need assistance in daily living but are not working toward skill development goals.
IHS With Training
Support combined with skill-building to increase independence in one or more areas of daily living.
IHS With Family Training
Services that include training and guidance for family members to better support their loved one’s independence and growth.
