Community Access for Disability Inclusion (CADI)

Community Access for Disability Inclusion (CADI) Waiver is a Minnesota Medicaid waiver program designed to support individuals with disabilities who require the level of care typically provided in a nursing facility, but who choose to live in their own homes or other community-based settings.

The CADI Waiver provides flexible services that help individuals remain connected to their communities while receiving the support necessary to live safely and confidently. It serves individuals with physical, intellectual, or developmental disabilities who benefit from structured, personalized care.


Services May Include:

  • Case Management

  • Personal Care Assistance

  • Home Modifications

  • Respite Care

  • Employment Support

  • Adult Day Services

  • Specialized Equipment and Supplies

  • Behavioral Support

Eligibility

To qualify for the CADI Waiver, an individual must:

  • Be under 65 years of age

  • Require a nursing facility level of care

  • Be eligible for Medical Assistance (MA)

  • Choose to live in a community-based setting rather than an institutional facility
